Preparing your documents before your tax appointment helps make the process smoother, more accurate, and more efficient. Use this checklist to gather the information that applies to your household.

IDENTIFICATION AND PERSONAL INFORMATION

Bring a valid government-issued photo ID; Social Security cards or ITIN documents for everyone included on the return; dates of birth for you, your spouse, and dependents; your current contact information; and any Identity Protection PIN issued by the IRS.

INCOME DOCUMENTS

Gather all W-2 forms, 1099 forms, Social Security benefit statements, records of self-employment or business income and expenses, and any other documents showing income received during the year.

DEPENDENTS AND FAMILY INFORMATION

Bring Social Security cards and birth dates for all dependents, childcare provider information, education expense records, Form 1098-T when applicable, and any documents supporting your right to claim a dependent.

HEALTH INSURANCE INFORMATION

Bring Form 1095-A if you or anyone in your household had Marketplace health insurance. Include any other health insurance documents that may affect your return.

EXPENSES AND TAX BENEFITS

Gather mortgage interest and property tax statements, charitable contribution records, qualifying medical expenses, business mileage and expense records, student loan interest statements, retirement contribution information, and estimated tax payment records.

REFUND INFORMATION

Bring your bank routing and account numbers for direct deposit and a copy of your previous year’s tax return, if available.

Not every item will apply to every Purpose Partner. If you are unsure whether a document is needed, bring it with you or contact On Purpose 4 U before your appointment.
